On April 18, 2005 two HCSF surcharge rates were corrected. Both
corrections were in the $100,000/$300,000 HCSF Coverage Limits table. The
fourth year Fund Class Group 5 surcharge rate was changed from $1,112 to the
correct surcharge amount of $1,125. The third year surcharge Fund Class Group
surcharge rate was changed from $205 to the correct surcharge amount of $203.)
