Neoplastic Diseases, Nephrology, Neurology
(including child), Ophthalmology (including minor
and major surgery), Otology, Otorhinolaryngology,
Pathology, Pediatrics, Radiology, Rhinology, Shock
Therapy and other Physicians who are performing
minor surgery and are not otherwise
classified.
